No, Custom fit. Like I said...more often than not, thick neck = short arms because the manufacturers assume that said person with a thick neck is thicker than someone with a thin neck. If I buy Thick-neck long sleeve, I usually have to get them altered for fit. I basically hate the way clothes fit off the rack most of the time. Which probably has something to do with my former overweight self-consciousness.
I have one or two shirts I wear with suits only that are monogrammed and are custom tailored though.

Again, you realize that any respectable dress shirt doesn't come in xl,L,etc denominations. They all have a neck and sleeve length combination that come in virtually every combo. And if you are buying something that is designed for 'slim fit' vs. 'normal' then you'd be just fine.
But you don't know that, because you probably buy everything off the rack...as Toast apparently has seen personally.
